The Center for Sports Medicine & Orthopaedic Surgery (CSMOS) is pleased with the decision by you and your surgeon to utilize our facility. Our staff wants to make your upcoming visit as comfortable and pleasant as possible.

Our goal is to provide high quality and cost-effective, orthopaedic surgery, sports medicine, and pain management care to the community with an emphasis on service to our patients, patient’s families, physicians, and employees. We are certified by the Centers for Medicare and Medicaid Services (CMS) and accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC). Outside organizations can quickly and easily request the verification of physicians in good standing at our facility. The requesting organization may self generate and print a verification letter.

Our rigorous credentialing process follows Federal and State regulatory requirements, as well as accreditation standards for verifying the education, training, licensure and current competence in order to ensure that privileges are only granted to qualified Providers.
